OCD Treatment in Multnomah and Portland, OR
OCD is often misrepresented as a quirk about cleanliness or organisation. For people who actually have it, the experience is different. It involves intrusive, unwanted thoughts that cause significant distress and compulsive behaviours that feel necessary to relieve that distress, at least temporarily. It can take over hours of the day and make ordinary life very difficult. At Aspirant Wellness Solution we provide psychiatric evaluation, medication management, and ERP therapy referrals for OCD in Multnomah, Portland, and across Oregon through telehealth.

Harm OCD
Intrusive thoughts about accidentally or intentionally harming yourself or others

OCD presents very differently from person to person. If intrusive thoughts and compulsive responses are taking up significant time or causing significant distress, that is worth evaluating.
What OCD Actually Is and Why It Is Often Misunderstood
Obsessive-compulsive disorder involves two core components. Obsessions are unwanted, intrusive thoughts, images, or urges that cause significant anxiety or distress. Compulsions are repetitive behaviours or mental acts performed to neutralise that distress. The relief from a compulsion is temporary, which is what keeps the cycle running.
A common misconception is that OCD is about being neat or organised. In reality, OCD is an anxiety-spectrum disorder. The content of the obsessions can be anything, including things that feel completely at odds with who the person is as a human being. That disconnect is actually a hallmark of OCD rather than evidence of real intent or desire.
OCD is treatable. The most effective approach combines medication, typically SSRIs at higher doses than are used for depression, with exposure and response prevention therapy. We manage the medication side and refer to ERP-trained therapists in the Portland and Multnomah area when therapy is indicated.

- Doubt about whether an action was completed
- Religious or moral intrusive thoughts
- Repeated washing or cleaning

- Seeking reassurance from others
- Mental reviewing or neutralising thoughts
- Ordering or arranging until it feels right
- Confessing or praying to undo feared harm
OCD Presentations We See and Treat in Multnomah and Portland
OCD does not look the same in every person. The content of obsessions varies widely, and so do the compulsions. Here are the presentations we most commonly evaluate and treat.
Contamination OCD
Driven by fear of germs, illness, or contamination. Compulsions typically involve washing, cleaning, or avoiding anything perceived as contaminated. Can become severely debilitating when the avoidance spreads to larger areas of daily life.
Harm OCD
Intrusive thoughts about harming oneself or others, despite no actual desire or intent to do so. The content of these thoughts feels completely alien to who the person is, which causes significant distress. People with harm OCD are not dangerous. The thoughts are ego-dystonic, meaning they are experienced as unwanted and contrary to the person's values.
Checking OCD
Compulsive checking of locks, stoves, switches, or actions taken. The person knows intellectually they have checked, but the doubt returns and the checking starts again. The compulsion temporarily reduces anxiety but reinforces the cycle.
Pure Obsessional OCD
Sometimes called Pure O. Characterised by primarily mental obsessions without obvious external rituals. The compulsions tend to be internal, such as mental reviewing, reassurance-seeking, or thought suppression. Often misdiagnosed because the compulsive component is not visible.
Symmetry and Ordering OCD
A need for things to be arranged or aligned in a specific way. Accompanied by a sense that something is wrong or incomplete until it is just right. This is not about preference or aesthetics. The discomfort driving the behaviour is real and significant.
Scrupulosity
Religious or moral OCD. Obsessions centre on whether a person has sinned, behaved immorally, or offended God. Compulsions include excessive confession, prayer, or reassurance-seeking. Can conflict significantly with a person's faith and cause serious distress.
Medication Management and ERP Referral for OCD in Portland
OCD responds best to a combination of medication and a specific type of therapy called exposure and response prevention, or ERP. We manage the medication side of that equation and connect patients with ERP-trained therapists in the Portland and Multnomah area.
Step 1: Psychiatric Evaluation
We assess the type of OCD you have, how severe it is, how much time it takes up each day, and how it affects your functioning. We also check for common co-occurring conditions like depression and anxiety.
Step 2: Diagnosis and Psychoeducation
We confirm the OCD diagnosis and explain clearly what OCD is and how it works. Understanding the cycle of obsessions and compulsions is actually an important part of treatment, not just a preliminary step.
Step 3: Medication Management
SSRIs are the first-line medication for OCD, typically at higher doses than used for depression. We start at a lower dose and increase gradually. Clomipramine is used when SSRIs do not provide sufficient relief. We monitor response and adjust carefully.
Step 4: ERP Referral
Exposure and response prevention therapy is the gold standard psychological treatment for OCD. We refer to ERP-trained therapists in Portland and Multnomah for patients who would benefit from therapy alongside medication.
Medications Used for OCD
Sertraline, fluoxetine, fluvoxamine, paroxetine, escitalopram. Effective for OCD at doses that are typically higher than those used for depression or anxiety. Full effect takes 8 to 12 weeks at the therapeutic dose.
A tricyclic antidepressant with strong anti-OCD properties. Used when SSRIs have not provided enough relief. Requires monitoring because of its side effect profile, but it is highly effective for treatment-resistant OCD.
When a single medication does not fully reduce symptoms, adding a low-dose atypical antipsychotic such as risperidone or aripiprazole can improve response. Used in treatment-resistant cases.
OCD typically requires higher SSRI doses than depression. Starting low and increasing gradually is standard. Adequate response may take 8 to 12 weeks at the right dose, so patience with the process is part of the plan.
What Else Often Comes With OCD
Depression is extremely common in people with OCD, largely because living with constant intrusive thoughts and compulsions is exhausting and demoralising. Anxiety disorders also frequently co-occur with OCD, though OCD is its own distinct condition rather than a type of anxiety disorder. ADHD is also seen alongside OCD more often than chance would predict.
We take co-occurring conditions seriously in the evaluation and build a treatment plan that accounts for them. Treating OCD while missing an underlying depression, for example, tends to produce incomplete results.
- OCD and depression: both can be addressed with medication, often with overlapping benefit
- OCD and anxiety: OCD is related to anxiety but requires a different treatment approach
- OCD and ADHD: attention difficulties can complicate OCD management and need to be accounted for
- OCD and body dysmorphic disorder: BDD is closely related to OCD and may occur alongside it

Conditions Related to OCD That We Also Treat
These conditions share features with OCD or commonly occur alongside it.
What is the difference between OCD and regular anxiety?
Anxiety disorders involve fear and worry about real or perceived threats. OCD involves specific intrusive thoughts, images, or urges that cause distress, combined with compulsive behaviours performed to neutralise that distress. The two often co-occur, but OCD is classified separately and treated differently. SSRIs for OCD are used at higher doses than for anxiety, and ERP therapy is specific to OCD treatment.
What medications are used to treat OCD?
SSRIs are the first-line medication for OCD. Sertraline, fluoxetine, fluvoxamine, and paroxetine are commonly used. Doses for OCD are typically higher than for depression, and it can take 8 to 12 weeks at a therapeutic dose to see full effect. Clomipramine is used when SSRIs do not provide enough relief. Augmentation with a low-dose antipsychotic is sometimes added in treatment-resistant cases.
What is ERP therapy and do I need it?
Exposure and response prevention therapy is the most effective psychological treatment for OCD. It involves gradually facing situations that trigger obsessions while resisting the urge to carry out compulsions. The goal is to break the obsession-compulsion cycle over time. Medication reduces the intensity of symptoms, which makes ERP more manageable. We refer to ERP-trained therapists in the Portland and Multnomah area.
Can OCD be treated through telehealth in Oregon?
Yes. We offer telehealth appointments for OCD evaluation and medication management to patients anywhere in Oregon. We can also coordinate ERP referrals for patients outside the Portland metro area.
How long does OCD treatment take?
OCD is a long-term condition for most people. Medication helps reduce symptoms but typically does not eliminate them entirely on its own. ERP therapy, combined with medication, produces the best outcomes over time. Most people see meaningful improvement within a few months of starting the right combination, though managing OCD is an ongoing process rather than a short course of treatment.
Does insurance cover OCD treatment in Portland?
Most major Oregon insurance plans cover psychiatric evaluation and medication management for OCD, including Oregon Health Plan. We verify your coverage before your first appointment.
