What ODD Is and How It Develops
ODD is defined by a persistent pattern of angry or irritable mood, argumentative and defiant behaviour, and vindictiveness lasting at least six months, and to a degree that is excessive for the child’s age and development. It is not just difficult behaviour. It is a clinical condition that interferes significantly with functioning at home, at school, or with peers.
ODD rarely exists on its own. ADHD is present in roughly half of children with ODD, which makes sense because emotional dysregulation and impulse control problems drive both. Anxiety is also frequently underneath what looks like defiance. A thorough evaluation matters because treating ADHD or anxiety often reduces ODD symptoms considerably, without ODD being the primary treatment target.
We work with the young person and their family. Parent training and family involvement are important parts of effective ODD treatment, and we make recommendations that include the family context.

Evaluation and Treatment for ODD in Portland

Step 1: Full Evaluation
We assess ODD symptoms, ADHD, anxiety, mood, family context, and school situation. We take a full history and talk with the young person and family together and separately.
Step 2: Identify Drivers
We determine whether ADHD, anxiety, depression, or other conditions are underlying or contributing to the oppositional behaviour. Treating those conditions is often the most effective way to reduce ODD symptoms.
Step 3: Medication if Needed
If ADHD or anxiety is a significant driver, treating those with medication often improves ODD symptoms substantially. There is no medication specifically for ODD, but medication for co-occurring conditions helps considerably.
